It is with much regret that Dr Chakrabarti is announcing his retirement from  Private Practice. He has ceased practising after seeing his last patients on 07.08.2020. This decision has been a very hard one after practicing in the area for 20 years. 

Over the last year Dr Chakrabarti has been transferring care of patients to other mental health professionals  in order to maintain patients' ongoing care. However, there may be some patients that have either not attended recently or may have missed their appointments. If you have not been able to discuss ongoing management options with Dr Chakrabarti please make an appointment with your GP and explore referrals to other psychiatrists in your region. Please be aware that it can sometimes take 6-8 weeks or longer to avail of specialist appointments and so the earlier you meet your GP the earlier the process will start. While waiting for your initial appointment with a psychiatrist you may wish to see any other mental health professional that you may have already been involved with more frequently.

It would be quite beneficial to remind your GP to send all Dr Chakrabarti's clinical communications to the new Psychiatrist along with the referral. If you have seen Dr Chakrabarti recently then a clinical summary would have been sent out to your GP.

Dr Chakrabarti would like to wish all patients well for their futures and would like to thank the families that have been so involved over the years.
